What is social prescribing?
“Social Prescribing, sometimes referred to as community referral, is a means of enabling GP’s, nurses and other primary care professionals to refer people to a range of local, non clinical services.” – NHS
Social Prescribing is not in place of clinical services, but in addition to.
The service is here to support those who:
- Are Adults who are aged 18 + and registered with a Bury GP
- Are Lonely or isolated
- Have social needs that affect their well-being
- Need support with their mental health
- Have one or more long term condition
